How To File Warranty Claim For Vet Bills
In the unfortunate event that you need to file a claim, please fill out the fields below and attach the following.
A copy of your Warranty Contract, Initial Exam, and an itemized veterinary bill.
Please ensure that the itemized bill from the treating veterinarian lists the identification for your pet, the procedures that were performed, and a diagnosis.
